Students must have proof of all required NYS immunizations on file. Students who are not up to date may be excluded from school after the 14th day unless they are documented as “in process.” Students transferring from out of state or another country have 30 days to provide immunization records.
NYS requires current physical exams for students entering Pre-K/K, 1st, 3rd, 5th, 7th grade, and all new students. Please submit your child’s completed physical form to the Health Office by the start of the school year.
All prescription and over-the-counter medications require completed parent/guardian and healthcare provider documentation. Medications must be provided to the School Nurse in the original, pharmacy-labeled container. Students may not carry or self-administer medication without written authorization.
If your child has an injury or medical condition requiring activity restrictions, please provide a healthcare provider’s note outlining the restrictions and return-to-activity date. Submit documentation to the School Nurse as soon as possible.
Communicable Illnesses
Please notify the School Nurse if your child is diagnosed with a contagious illness such as flu, strep, chickenpox, or pink eye. A healthcare provider’s clearance is required before returning to school. Students with a fever should remain home until they are fever-free without medication.
Sports Participation
Students must have a current NYS physical on file to participate in school sports. If the physical is more than 30 days old at the start of the season, a parent/guardian must complete an updated health history form. Students who may need rescue medication, such as an inhaler, must also have the appropriate medication forms completed. Please submit all required health documentation before the sports season begins.
